Clinical Trial on Treatment of Rheumatiod Arthritis with Recombinant Interferon

Rheumatoid arthritis (RA) is chronic systemic disease whose pathogenesis is not clarified yet. It is an autoimmune disease characterrized by chronic inflammation of synovium of synovium which is caused by abnormality of humoral and cellular
immune
response. It is reported that deficiency or overproduction of many kinds of cytoines give rise to pathogenesis of many autoimmune disease, Based on this observation, there has been numerous attempts to make use of various cytokines in the
treatement of
autoimmune disease. IFN-γis cytokine secreted from activated T lymphocyte and has antiviral activity antiproliferative and immunomodulatory activities. This reserch covers 126 RA patients, and tested clinical efficacy and safety of IFN-γby
trying
0.5×10E6-1 IU/day each. Of 126 patients, 113 patients are evaluated: 64 responded (56.6%), and the others didn't (43.4%). In response group, Ritchie articular index and Lansbury index decreased to 44%, 43% respectively at the fourth-week
evaluation.
Ritchie index and Lansbury index checked on 14th day, are likely to be used as an early indicator judgement of treatement becreased more meaningfully in response group than in non-response group. Morning pain, movement pain, grip strength and
morning
atiffness are felieved considerably in response group compared to non-response group. Westsergren ESR and CRPare improved in response group, but there are no significant differences. There are no differences between two groups in the results of
treatement with respect to the charateristics of patients and the previous of concomitant medication. There has been no change in leukocyte, platelet, hemoglobin and liver function before and after treatment, also between two groups. It is shown
that 44
patients among 113 have side effects. The most frequent one is fever-30 cases, chilling, sweating, headache-7, 6, 5 cases each, dizziness, heating sense, hypotension, fatigue-2 cases repectively, local irritation, cyanosis, leukopenia-1 case
respectively 11 cases among 126 cases must be stopped due to side effects-7 cases fever, 2 cases hypotension, 1 case cyanosis and leukopenia each In conclusion, It seems that IFN-γ is effective in the treatment of RA patients who do not respond
to
DMARD Further clinical study on the long term treatment is required.
